Our staff work closely with the multi-disciplinary team, to ensure they have the right skills and approach to provide a service designed for your needs. We facilitate meaningful engagement and are committed to providing support that will give you good outcomes and life opportunities.

Person-centeredness is at the core of what we do. We recognise and value people’s experiences. People in our services have the right to express their concerns and have access to external agents, who will act in their best interests to address and solve problems in their lives. We will provide support to explore all options through partnership working with local advocacy groups and other support networks.

Seamless Transitions

The main purpose of transition planning and support process is, to provide complete planning for the transition of people into our service. This will ensure that we work within an agreed time frame.

Inclusion

People are included rather than excluded from participating in their community. We do this by improving awareness of local amenities and activities that might be of interest to them.

Transparency

People, parents and wider agencies will be involved in facilitating a smooth transition. We will work together to agreed achieve outcomes.

Communication

During the review process all relevant information is shared with agencies that are involved the person care and support. This is to ensure that we are all working to improve out comes while observing safeguarding protocols
